摘要
摘要目的探讨成都地区老年高血压患者糖及脂代谢异常情况.方法采取分层整群抽样调查成都地区６０－７９岁人群(１９７８例)一般基线情况,体格检查,及相关生化指标,并确定该人群糖脂代谢异常患病率.以是否患高血压分二组组间计数资料比较采用χ２检验,计数资料采用方差分析;采用多元logistic回归分析老年高血压相关危险因素.结果该人群高血压总体患病率５６．７２％,其中男性为５５．１６％,女性为５７．９８％.老年高血压人群IGT(糖耐量减低)(２９．２３％vs２４．８９％,P＜０．０５)、IGR(空腹血糖受损)(３１．４６％vs２７．８％,P＜０．０５)和DM(糖尿病)(３４．５８％vs１９．３９％,P＜０．０５)的患病率明显高于老年非高血压人群.老年高血压人群高TC(６．２４％vs５．２６％,P＜０．０５)、高TG(１９．６７％vs１２．９７％,P＜０．０５)、低HDL－C(１０．７８％vs７．９４％,P＜０．０５)的患病率明显高于非高血压组的老年人群.多因素Logistic回归分析显示,HDL－C升高为老年高血压的保护因素,年龄、BMI、LDL－C、餐后血糖和尿酸均为老年高血压的危险因素.结论成都地区６０－７９岁老年高血压人群糖脂代谢异常明显高于非高血压患者.关键词高血压;代谢;老年StudyofmetabolicfactorsinoldhypertensiveinChengduresidentsZhonghong,Jinhong,Huangxiaobo,Wangbinyou,Mayan(DepartmentofGeriatＧrics,TheSecondPeople’sHospitalofChengdu,Chengdu６１００１７,China)AbstractObjectiveTostudytheepidemiologicalstatusofmetabolicabnormalitiesinChengduelderlyhypertensionpatients．Methods１９７８peopleagedfrom６０to７９yearoldwithhypertensionwereenrolledinthisstudy．Theirbaselinecharacter,physicalexamination,biochemicaldataandprevalenceofmetabolicabnormalitieswereobtained．Studypopulationweredividedintotwogroupsaccordingtohypertensionstatus．DifferencebetweengroupswerecomparedandlogisＧticregressionwasuesdtoanalyzetherealtiveriskfactorsinelderlyhypertension．Resultsthetotalhypertensionprevalenceis５６．７２％inthesepeople(male５５．１９６％,female５７．９８％)．Inelderlyhypertensionpatients,prevalenceofIGT(２９．２３％vs２４．８９％,P＜０．０５),IGR(３１．４６％vs２７．８％,P＜０．０５),DM(３４．５８％vs１９．３９％,P＜０．０５)washigherthannonhypertensionpeople．Moreover,theprevalenceofhighTC(６．２４％vs５．２６％,P＜０．０５),highTG(１９．６７％vs１２．９７％,P＜０．０５),andlowHDL－C(１０．７８％vs７．９４％,P＜０．０５)washigherinelderlyhypertensionpatientsalso．Andthelogisticanalysisshowedthatage,BMI,LDL－C,postprandialglucoseanduricacidareriskfactorsofhypertensioninelderlypeople．ConclusionstheprevalenceofmetabolicabnormalitiesinChengduelderlyhyperＧtensionKepyatwieonrtdsswashigherthanwhichinnonhypertensionpeople．Hypertension;Metabolism;Elderly中图分类号R５８７．１文献标识码B文章编号１００１－５３０２(２０１５)０９－０８８０－０２
