简介:目的探讨人造心脏瓣膜行心脏瓣膜替换术的临床应用的效果。方法1985年7月~2001年9月应用人造心脏瓣膜进行瓣膜替换术420例,其中二尖瓣替换术(MVR)316例,主动脉瓣替换术(AVR)85例,二尖瓣及主动脉瓣双瓣替换术(DVR)19例,置入瓣膜439只。结果早期死亡29例(6.9%),其中MVR5.7%.AVR10.6%,DVR10.5%,死亡原因主要是低心排(LOS)、心律失常。随访0.5~16年,总随访时间2561.66病人·年(平均6.98病人·年),晚期死亡12例,晚期死亡率为0.47%病人·年,死亡原因主要是伴发病与抗凝问题。结论心脏瓣膜替换术随着体外循环技术的改善,死亡逐年下降,尤其是近年保留瓣膜及瓣下结构的瓣膜替换以后。国产瓣仍不失为一种良好的瓣膜选择。
