简介:目的利用Micro-CT扫描数据建立中耳听骨链三维有限元模型。方法用Micro—CT扫描成人颞骨标本,将获得的图像数据通过MicroView、Mimics等软件进行三维重建。再将模型转入其有限元分析(finiteelementanalysis,FEA)模块的Remesh环境中进行调整、细化及面网格优化,通过SOLIDEWORK软件转换为实体网格。结果初步形成的三维几何模型可较清晰地辨别鼓室腔、听小骨和内耳系统,但部分图像不同程度存在噪点。最终形成了FEA软件可识别的成人中耳听骨链三维有限元网格模型,网格划分后的听骨链有限元模型中,完整听骨链的节点数降低,由原来包括805个不合理节点在内的12498个节点降低到2050个,在此基础上建立的有限元模型共1350个8节点四面体单元。结论结合Micro—CT技术及Mimics软件的三维建模方法可以快速获得较精确的听骨链三维数据,是建立听骨链三维有限元模型的有效途径。(中国眼耳鼻喉科杂志,2009.9.83。85)
简介:AIM:Toevaluatetheefficacyandsafetyoftrabeculectomy,phacotrabeculectomyplusintraocularlensimplantation(phacotrab+IOLgroup)andphacoemulsificationwithIOL(phaco+IOL)inprimaryangle-closureglaucoma(PACG).METHODS:Itwasasystematicreviewandmeta-analysis,randomizedcontrolledtrials(RCT)andclinicalcontrolledtrials(CCT)werecollectedthroughelectronicsearchesoftheCochraneLibrary,PubMed,EMbase,WanfangDatabaseonline,ChinesejournalFull-textDatabase,ChineseScientificJournalsFull-textDatabase(fromthedateofbuildingthedatabasetoOctober2010)Wealsocheckedthebibliographiesofretrievedarticles.Alltherelateddatathatmatchedourstandardswereabstracted.ThequalityofincludedtrialswasevaluatedaccordingtotheDutchCochraneCentre.RevMan5.0softwarewasusedforMeta-analysis.RESULTS:Atotalof5RCTand11CCTinvolving1495eyeswereincluded.Theresultsofmeta-analysisshowedthatphacotrab+IOLgroupwassuperiorthantrabeculectomy(trabgroup)(MD-3.93,95%CI[-7.31,-0.54])whichwasalsosuperiorthanphaco+IOLgroup(MD0.52,95%CI[0.10,0.95])indecreasingIntraocularPressure(IOP).Phacotrabgroup(MD-1.45,95%CI[-1.68,-1.22])andphacogroup(MD-1.12,95%CI[-1.87,-0.37])arebothdeeperthantrabgroupintheanteriorchamberdepth.Inincreasingthecoefficientofoutflowfacilityofaqueoushumor(Cvalues)therewasnostatisticaldifferenceinthethreegroups.Andtherewasnostatisticaldifferencebetweenphacotrabgroupsandphacogroupsinvisualacuitybutphacotrabgroupwassuperiorthanphacogroup(MD1.07,95%CI[0.73,1.40])intheuseofIOP-loweringdrugs.Therewasnostatisticaldifferenceamongthreegroups.CONCLUSION:Currentevidencesuggeststhatphacotrab+IOLgroupwassuperiorthantrabgroupwhichwasalsosuperiorthanphaco+IOLgroupindecreasingIOP.Phacotrabgroupandphacogrouparebothdeeperthantrabgroupintheanteriorchamberdepth.PhacotrabgroupwassuperiorthanphacogroupintheuseofIOP-loweringdrugs.
简介:目的研究不同程度面神经损伤后面神经运动神经元细胞凋亡相关蛋白caspase-3的表达。方法制作面神经切断和阻断30s的Wistar大鼠面瘫模型,应用链霉素卵白素生物素复合物检测术后1d至4周面神经运动神经元细胞凋亡相关蛋白caspase-3的表达。结果面神经切断组面神经运动神经元caspase-3表达先上升后下降,其中切断后第7天最高,1—7dcaspase-3的阳性率为25%-62%,7—28dcaspase-3的阳性率为0~62%;面神经阻断组面神经运动神经元caspase-3的表达与面神经切断组相似,1—7dcaspase-3的阳性率为23%-58%,7—28dcaspase-3的阳性率为0~58%。结论caspase-3可反应面神经运动神经元的凋亡情况,提示可以利用调控凋亡相关蛋白的表达干预凋亡,达到治疗面神经损伤造成的面瘫。(中国眼耳鼻喉科杂志,2008,8:12.14)
简介:目的研究面神经损伤后对其运动神经元死亡的影响。方法以大鼠为研究对象,设计出5种面神经受损的实验模型,即面神经低位断伤,面神经高位断伤,面神经茎乳孔压榨伤,面神经茎乳孔牵拉伤,面神经茎乳孔切断后立即桥接。各组动物饲养1月后取其面神经核团,经CresylViolet染色,在显微镜下计算右,左侧面神经核团中运动神经元数目之比。结果实验表明:面神经低位断伤,高位断伤,压榨伤,牵拉伤,切断后立即桥接其运动神经元胞体存活率分别为78.26%,69.23%,94.02%,81.58%和83.85%,面神经低位断伤后运动神经元存活率同其它各组比较发现:面神经低位断伤后其运动神经元存活率比高位断伤者高,较面神经压榨伤和面神经切断后立即桥接低,同面神经牵拉伤者没有明显差异。结论面神经受损后其运动神经元数目的变化同损伤部位,损伤程度,以及损伤后的修复有关,为面神经损伤后手术时机的把握和手术方法的选择提供理论依据。
简介:目的探讨边缘系统兴奋对耳鸣状态下听觉传导主要核团下丘外核(extemalnucleusofinferiorcolliculus,ICx)放电的影响。方法采用细胞外记录方法,观察急性水杨酸(sodiumsalicylate,SA)耳鸣模型大鼠杏仁外侧核(lateralamygdaloidnucleus,LA)自发放电及电刺激LA后ICx自发放电活动和放电间隔直方图。结果1.SA模型大鼠LA神经元平均自发放电率较正常对照组增加(5.42±0.68Hzvs3.36±0.24Hz,P〈0.01)。2.电刺激LA2h后,正常对照组大鼠的ICx平均自发放电率较刺激前增加(7.90±0.85Hzvs3.21±0.41Hz,P〈0.01);0~50ms间隔放电脉冲比例由31.9%增加到55.1%。3.急性SA模型大鼠在电刺激LA前,ICx的平均自发放电率(7.63±0.70Hz)已高于正常对照组,其中0—6ms短间隔放电比例明显增加;刺激LA后2h内,ICx神经元平均自发放电率降低(5.66±1.05Hz)与电刺激前比有差异(P〈0.05),但较正常组刺激前,自发放电活动仍明显增加;0~20ms的ICx自发放电脉冲数比例明显下降(28.4%vs14.5%)。结论大剂量SA可引起LA和ICx神经元放电增加;SA引起的LA神经元兴奋可引起ICx兴奋程度的改变,耳鸣是否伴随情绪因素,有待进一步研究。
简介:AIM:Toassesstheeffectivenessofimmunosuppressantsintheprophylaxisofcornealallograftrejectionafterhigh-riskkeratoplastyandnormal-riskkeratoplasty.METHODS:WesearchedtheCochraneCentralRegisterofControlledTrials(CENTRAL),MEDLINE,EMBASE,CNKI,VIPandreferencelistsofarticles.Dateofmostrecentsearch:18June,2011.Allrandomisedcontrolledtrials(RCTs)assessingtheuseofimmunosupressantsinthepreventionofgraftrejection,irrespectiveofpublicationlanguage.Twoauthorsassessedtrialqualityandextracteddataindependently.Onlydichotomousoutcomes(cleargraftsurvival,ratioofimmunereactionsandsideeffects)wereavailableandwereexpressedasrelativerisk(RR)and95%confidenceintervals(CI).RESULTS:Sevenstudieswereincludedinthisreview.Inthecomparingofmycophenolatemofetil(MMF)withplacebo,theresultsshowedMMFcouldsignificantlyreduceimmunereactionscomparedwithplacebo(RR1.0895%Cl0.95to1.21),butnoeffectoncleargraftsurvival(RR1.1195%Cl0.90to1.35).Incleargraftsurvivalandimmunereactions,MMFandcyclosporineA(CsA)showedsimilareffect(RR1.1195%Cl0.90to1.35,andRR1.48,95%Cl0.56to3.93,respectively).Tacrolimus(FK506)andsteroidshowedsimilareffectsoncleargraftsurvivalandimmunereactions(RR0.32,95%CI0.02to6.21,andRR1.00,95%CI0.88to1.14,respectively).Nodrugrelativesideeffecthasbeenfound.CONCLUSION:MMFmayreduceimmunereactionsinbothnormal-riskandhigh-riskrejectionofpenetratingkeratoplasty.CsAandFK506showedsimilareffectsasMMF.However,duetothelackoflargeclinicaltrials,theevidenceremainweak,thequalityofevidenceswereratedasverylowtomoderate.Large,properlyrandomised,placebo-controlled,doublemaskedtrialsareneededtoevaluatetheeffectofimmunosuppressants.
简介:目的:分析维吾尔族与汉族近视患者中央角膜厚度(centralcornealthickness,CCT)是否存在差异,了解维吾尔族近视患者中央角膜厚度与角膜曲率、眼压值及屈光度关系。方法:对屈光中心就诊近视患者汉族137例274眼及维吾尔族患者115例230眼测量其角膜中央厚度、眼压、屈光度及角膜曲率,分析维吾尔族与汉族CCT差异性及维吾尔族CCT与角膜曲率、眼压值及屈光度关系。结果:维吾尔族CCT(514.94±31.94μm)与汉族CCT(514.86±29.50μm)无显著性差异(t=0.029,P=0.977)。维吾尔族近视患者CCT与眼压间差异有统计学意义(r=0.4944,P〈0.01)。维吾尔族近视患者CCT与角膜曲率及屈光度间差异无统计学意义(r=0.093,P〉0.05;r=0.120,P〉0.05)。结论:维吾尔族与汉族CCT无显著性差异,维吾尔族近视患者角膜厚度与眼压间存在相关性,而与角膜曲率及屈光度无相关性。
简介:AIM:Toevaluatetheefficacyandsafetyofcornealcollagencrosslinking(CXL)topreventtheprogressionofpost-laserinsitukeratomileusis(LASIK)cornealectasia.·METHODS:Inaprospective,nonrandomized,single-centrestudy,CXLwasperformedin20eyesof11patientswhohadLASIKformyopicastigmatismandsubsequentlydevelopedkeratectasia.Theprocedureincludedinstillationof0.1%riboflavin-20%dextranesolution30minutesbeforeUVAirradiationandevery5minutesforanadditional30minutesduringirradiation.Theeyeswereevaluatedpreoperativelyandat1-,3-,6-,and12-monthintervals.Thecompleteophthalmologicexaminationcompriseduncorrectedvisualacuity,bestspectacle-correctedvisualacuity,endothelialcellcount,ultrasoundpachymetry,cornealtopography,andinvivoconfocalmicroscopy.·RESULTS:CXLappearedtostabiliseorpartiallyreversetheprogressionofpost-LASIKcornealectasiawithoutapparentcomplicationinourcohort.UCVAandBCVAimprovementswerestatisticallysignificant(P<0.05)beyond12monthsaftersurgery(improvementof0.07and0.13logMARat1year,respectively).Meanbaselineflattestmeridiankeratometryandmeansteepestmeridiankeratometryreduction(improvementof2.00and1.50diopters(D),respectively)werestatisticallysignificant(P<0.05)at12monthspostoperatively.At1yearafterCXL,meanendothelialcellcountdidnotdeteriorate.Meanthinnestcorneapachymetryincreasedsignificantly.·CONCLUSION:Theresultsofthestudyshowedalong-termstabilityofpost-LASIKcornealectasiaaftercrosslinkingwithoutrelevantsideeffects.Itseemstobeasafeandpromisingproceduretostoptheprogressionofpost-LASIKkeratectasia,therebyavoidingordelayingkeratoplasty.
简介:AIM:TocomprehensivelyevaluatethepotentialassociationofCOL1A1polymorphismswithhighmyopiabyasystematicreviewandMeta-analysis.METHODS:AllassociationstudiesonCOL1A1andhighmyopiareporteduptoJune10,2014inPubMed,Embase,WebofScience,andtheChineseBiomedicalDatabasewereretrieved.Oddsratios(ORs)and95%confidenceintervals(95%CIs)wereanalyzedforsinglenucleotidepolymorphisms(SNPs)usingfixed-andrandom-effectsmodelsaccordingtobetween-studyheterogeneity.PublicationbiasanalyseswereconductedbyEgger’stest.RESULTS:Atotaloffourstudiesfromreportedpaperswereincludedinthisanalysis.TheMeta-analysesforCOL1A1rs2075555,composedof2304highmyopiapatientsand2272controls,failedtodetectanysignificantassociationwithhighmyopia.Atotalof971casesand649controlsweretestedforCOL1A1rs2269336.TheassociationofCOL1A1rs2269336withhighmyopiawasobservedinrecessivemodel(CCvsCG+GG,P=0.03)andinheterozygousmodel(CGvsGG,P=0.04),butnotinothermodels.CONCLUSION:ThisMeta-analysisshowsthatCOL1A1rs2269336(CCvsCG+GG)affectsindividualsusceptibilitytohighmyopia,whereasthereisnoassociationdetectedbetweenSNPsrs2075555andhighmyopia.Giventhelimitedsamplesize,furtherinvestigationsincludingmoreethnicgroupsarerequiredtovalidatetheassociation.
