简介:AbstractObjective:The prevalence of midline birth defects, such as gastroschisis, has increased worldwide, over the last few decades. This study aims to explore the prevalence, maternal epidemiological characteristics, and natural history of neonates affected by gastroschisis at the University Hospital of León city, Nicaragua.Methods:Data were collected from the birth defect surveillance system of the Hospital Oscar Danilo Rosales (HEODRA). The analysis included all pregnancies that had gastroschisis complications between January 1 and December 31, 2020. The prevalence of gastroschisis was calculated according to maternal age. The mothers were interviewed, and the clinical records of the newborns were reviewed.Results:Among the 4,460 deliveries included in this study, four cases of gastroschisis were identified, including three live births and one stillbirth. The gastroschisis rate was 8.9 per 10,000 live births (95% confidence interval [CI]: 0.18-17.8). The prevalence among mothers younger than 20 years and those older than 20 years was 26.4 (95% CI: -3.43 to 56.25) and 3.01 (95% CI: 2.89-8.90)/10,000 births, respectively. Mothers of gastroschisis-affected fetuses were of rural origin (n = 3), had normal body mass indexes (n = 3), were exposed to tobacco and wood smoke (n = 2), and one was exposed to pesticides during the periconceptional period. Primary closure of the gastroschisis was performed on one patient, and complex gastroschisis for intestinal perforation was observed in another patient. The mean hospitalization duration was 33 days, and two patients were discharged alive.Conclusions:Gastroschisis was a significant birth defect among children delivered at HEODRA in 2020. Its prevalence in Nicaragua was higher than that in other countries in the region. All complicated pregnancies were young women with unplanned pregnancies, from rural areas, with exposure to secondhand smoke, and without vitamin supplements before or during the first trimester of pregnancy. Only 67% of infants survived after hospital discharge.

简介:摘要 目的:了解临床N0-N2级护士对生存质量的认知状况及其影响因素,从而促进护理质量的提高并起到推动优质护理服务的作用。方法:采用自设问卷,对我院和某三甲医院心血管内科、呼吸科、肾内科、消化内科、神经内科、肿瘤科、内分泌科、脑外科、肝胆外科和骨伤科护士进行问卷调查,内容包括一般资料、护士对生存质量的认知和态度以及领床护士开展评价生命质量的困难等。结果:①30.0%的护士知道生命质量,52.8%的不知道生存质量的概念,并且有58.6%的不知道目前国内生命质量的研究方向。②在临床上有81.4%从没有运用过生命质量。③56.5%很愿意进一步了解学习生命质量,并且有51.0%希望通过专题讲座的方式来学习。

简介:摘要目的明确鼻咽癌N0-N1期患者Ⅱb区淋巴结预防照射剂量CTV60Gy优化至CTV50Gy的安全性以及对腮腺保护的剂量优势和临床价值,了解PET-CT及弥散加权成像(DWI)对颈部可疑阳性淋巴结(最大横断面短径≥5 mm但<10 mm)的诊断价值。方法回顾性分析2015年6月至2017年3月收治的N0-N1期初治非转移鼻咽癌157例,其中104例在多模态影像指导下进行上颈部Ⅱb区临床靶区优化,以Kaplan-Meier法进行生存分析,单因素、多因素回归分析颈部淋巴结复发模式,配对t检验比较剂量优化前后靶区及腮腺剂量参数差异。结果N1期行对侧单颈Ⅱb区优化60例,行双颈Ⅱb区优化25例(仅咽后淋巴结转移者);N0期双颈Ⅱb区优化19例。共有3例患者出现颈部复发,均在高剂量区内。全组5年总生存率为93.3%,淋巴结无复发生存率、无局部复发生存率、无远处转移生存率和无瘤生存率分别为97.1%、91.3%、88.5%和80.8%。颈淋巴结复发与鼻咽局部复发有关,与咽后淋巴结状态无关。14例患者Ⅱb区内有可疑阳性颈淋巴结,最大横截面短径平均为7.1(5~9)mm,其中11例PET-CT阳性,平均SUVmax为2.96(2.5~3.3)。优化后肿瘤体积的差异无统计学意义(P>0.05),腮腺的Dmean、Dmax、D50%、V26Gy显著低于常规计划(P<0.01)。结论鼻咽癌N0-N1患者进行Ⅱb区CTV60Gy至CTV50Gy的剂量优化是安全的,腮腺及颈部周围正常组织受照剂量明显降低,对于未达诊断标准的小淋巴结需要结合PET-CT及DWI等多模态成像进行个体化处理。

简介:摘要:随着国际化进程的日益推进,国际间的合作越来越频繁,在我国的生产中国际材料的应用也越来越普遍。因此,对国际材料应用的研究是势在必行的。本文是对昆明地铁工程隧道设备盾构机盾体制作中所使用材料S355J2+N的焊接研究。本文通过对S355J2+N材料成分分析、焊接性分析、焊接试验等手段对S355J2+N厚板的焊接进行了可行性研究。为S355J2+N材料在以后生产中的应用提供了见解方案。
简介:AbstractAvian influenza remains a threat to human wellbeing. Hypochlorite derivatives are commonly used as disinfectants to prevent the spread of the disease. The World Health Organization (WHO) has listed chlorine dioxide (ClO2) as an A1-level, safe, and efficient disinfectant. In this study, we tested the efficacy of ClO2, in aqueous solution and gas forms, against avian influenza A (H7N9) virus. The virus suspension was mixed with ClO2 aqueous solutions of various concentrations and for various time intervals. Aliquots of the mixture were then serially diluted, and the 50% tissue culture infective dose (TCID50) was measured with a hemagglutination test on MDCK cells. ClO2 gas produced from generators was introduced in a chamber containing the virus suspension in a Petri dish. The infective activity of the surviving virus was measured by the hemagglutination test. An aqueous solution of ClO2 at 126 µg/mL for 15 s was effective given that no surviving virus was detected with the hemagglutination test. ClO2 gas at >5 µL/L sustained for 1 h inactivated the virus effectively, while at 2.5 µL/L for 1 h, it only partially inactivated the virus. ClO2 as gas or aqueous solution at a certain concentration is effective in inactivating the H7N9 virus, and can be applied for the decontamination and disinfection of environments.

简介:AbstractN-acetyl-cysteine (NAC) is an efficacious treatment for sensorineural hearing loss in animal models, such as noise-induced hearing loss (NIHL), however previous research into the effect of NAC on patients with hearing loss produced contradictory results. In this study, we investigated the effect of NAC treatment on sensorineural hearing loss. PubMed, Web of Science and Embase databases were searched in their entirety using the key words: hearing loss, NAC, N-acetylcysteine, and sensorineural hearing loss. Studies which included assessment of hearing loss with pure-tone threshold (PTA) data were selected. Eligible studies regarding the effects of NAC treatment on patients with hearing loss were collected by two independent reviewers. A total of 1197 individuals were included from seven published studies. Two studies reported data for a sudden idiopathic sensorineural hearing loss (SISNHL) group. Three studies reported data for a NIHL group. Other studies reported data for drug-induced hearing loss. The meta-analysis demonstrated that the overall effect of NAC treatment on sensorineural hearing loss was invalid. However, NAC treatment was linked with improved patient outcomes of hearing tests in cases of sudden hearing loss, but did not prevent hearing loss induced by noise or ototoxicity. However, there is a need for better-designed studies with larger samples to further prove the correlation between the effect of NAC and hearing loss.
